Start by figuring out what info you'll need for tracking costs and job progress. Without knowing your specific circumstances, I can't tell you exactly what you need, but you might have something like job number, description, date, employee or machine details, materials used, time spent, and any expenses.

Once you know what info you need to include on your job card, you can just make a basic spreadsheet or word processor (I like Libre Office) to make a template. It'd be like a fill in the blank kind of thing, like so:

Employee Name: ______________
Job Number: _____________
Description: _____________
Date: ___________________

...and so on.

There's really no set format. Just make sure you're asking for the information you'll need to record in your accounting software.

You could collected job name, date, workers involved, and materials used, for example. You can make a simple sheet in Excel that can be printed whenever you need. Have employees fill out the details each time work is done or materials are used. This helps you keep track of how much time and resources are spent on each job while the info is still fresh.

Just grab some paper or a spreadsheet, list out the job info, date, workers, and what they used or did. Keep it simple and update it whenever work happens. That way, you can monitor progress on each job. Getting the workers to remember to fill them out is another question.
